1. Complete all details on the application

2. Print the application form using your Internet Browser print button

3. Mail completed form, along with cheque, money order (payable to VCPS) or copy of direct deposit receipt (see item 6, below) to

4. Payment must be made in Australian Dollars only. Overseas Money Orders must be drawn on an Australian bank.

5. Credit card facilities (Visa & MasterCard) are available for non-Australian residing applicants only.

6. Australian residents can Direct Deposit membership funds into out bank account. Details are:

   • Commonwealth Bank, Bulleen Vic

   • BSB No. 063 583

   • Account Number 10038676

   • CBA International deposits. Swift Code CTBA AU 2S       (last character in code is S for Sally)

7. You will receive your membership card in the mail.
